Please drop in your Prescription Medication Card, or a list of all your medications with your name and address to the reception desk, for your repeat prescription, or click on the Request Prescription button on our home page
The reception team CANNOT take prescription requests over the phone.
Please mark the medications you require on your card, so that we can ensure you have your correct script. If your medication is changed by a consultant, or hospital, please inform us as soon as possible so that we can update your medical records, and ensure your next prescription is correct.
From time to time, as the Doctor reviews your prescriptions, you may be requested to attend the surgery for review of both your medications and yourself. In most cases a review is required every six months. Please allow AT LEAST 24 HOURS for us to prepare your prescription, as your medications and doses need to be checked, printed up and then signed by the doctor. All prescriptions must be collected from the surgery. We cannot phone or fax any prescriptions into any chemist, as all chemists need the prescription in paper format.
If you have a medical card, and get a hospital prescription, you are entitled to one week’s supply of your medications from your pharmacy when you produce the hospital prescription to them. You must bring this prescription to the pharmacy on the day the hospital issued you this script. Once you have your medication we would advise you to bring the hospital prescription to the surgery so that it can be filled out on GMS prescription paper and you will be able to receive the rest of your medications. Please note we need AT LEAST 24 HOURS to prepare this prescription.
MEDICATIONS CANNOT BE PRESCRIBED OVER THE TELEPHONE. THE PATIENT MUST BE SEEN IN THE SURGERY.
